- 10 Case Managers
- 3 Social Workers
- 2 CM Assistants
- 1 Manager
- 1 Director
- Typical caseload: 20–25 patients per RN; approximately 50 patients per Social Worker
- EMR: Meditech / Meditech Expanse (transition noted for 8/13)
- Equipment: Computer, phone, etc.
Required Experience
- RN license required
- Minimum 2 years of hospital Case Management experience
- No specific certifications required.
- Candidates without the required hospital CM experience will not be considered.
