Wound Care Specialist, Wound Care remote location. This role involves conducting health assessments focused on wound, ostomy, and continence-related care. The position includes developing and evaluating care standards, collaborating with the healthcare team, and providing patient education and staff training to optimize wound management and prevention strategies.
Job Details

Conducts health assessment interviews and performs focused physical assessments related to wound, ostomy, and continence care.

Develops, revises, and evaluates standards of care, assisting in the selection of supplies, technology, and equipment.

Assesses and documents patient response to wound care, ostomy, fistula, and complex wounds treatment.

Evaluates the quality and delivery of care provided by clinical staff.

Works collaboratively with the healthcare team and makes referrals to other disciplines as indicated.

Provides education, counseling, and assistance to individuals and families to prevent complications related to ostomies, wounds, or continence problems.

Develops, provides, and evaluates staff education programs such as new employee orientation, in-services, and continuing education.

Routinely provides comprehensive assessments of patients at risk for pressure injuries, initiating preventative measures.

Escalates and communicates challenges, including ethical issues and conflict resolution.

Collaborates with the Assistant Director of Nursing for Wound Care to initiate and recommend best practices based on assessments.

Participates in unit/department and hospital-wide quality improvement activities.

Provides culturally competent education to patients, families, and staff aligned with individual belief systems.

Maintains data security and confidentiality in accordance with patient rights policies.

Achieves and maintains professional competency through relevant clinical activities.

Identifies community resources helpful to clients, refers, and assists in utilization.

Asset management related to rented equipment such as specialty beds and negative pressure devices.

Required Skills & Experience:
Associates Degree (ASN, ADN) from an accredited Nursing Program, NYS RN License, AHA BLS certification, and a current certification in wound, ostomy, and continence nursing (e.g., CWOCN, CWON, CWCN, CWS, or WCC). Two (2) years of Med/Surg experience with wound and colostomy expertise. -
Schedule: Days shift, 7.5 hours per day, minimum 37.5 hours per week.

Additional Information:
Candidates should possess experience in wound and colostomy care, patient assessment, and care planning. Preferred skills include asset management, patient and staff education, quality improvement, and cultural competence.
