Qualifications

• Active Oregon or Washington RN license, or a compact RN license
• Minimum of 1 year of case management, care coordination, or Patient Care Coordinator experience
• BLS certification required
• Experience with EPIC charting preferred
• Experience working with Medicaid and Medicare populations preferred
• Previous experience in an outpatient support or healthcare coordination role preferred
• Willingness to float to a neighboring location as needed

Responsibilities

• Coordinate patient care needs and support efficient care delivery across the outpatient setting
• Collaborate with physicians, care teams, and support staff to promote continuity of care
• Document accurately in EPIC and maintain organized patient records
• Assist with patient flow management, scheduling support, and follow-up coordination
• Support patients with a variety of care needs while maintaining professionalism and compassion
• Work independently while contributing to a team-oriented healthcare environment
• Float to a neighboring location when needed to support coverage and patient care
