⏰ Schedule: 10:00 AM–7:00 PM or 11:00 AM–8:00 PM
⏱️ Guaranteed Hours: 40 hrs/week
Position Overview
We are seeking an experienced Hospice RN Case Manager to provide compassionate hospice care across San Francisco, San Mateo, and Santa Clara Counties.
The Hospice Nurse will assess patients and families, develop individualized plans of care, coordinate services with a multidisciplinary team, and provide follow-up care based on patient needs.
Responsibilities
Admit patients to hospice services
Assess patients' and families' physical, psychosocial, environmental, safety, and developmental needs
Develop individualized plans of care based on assessments
Document patient observations and nursing interventions
Coordinate care with the multidisciplinary team
Conduct follow-up visits as needed
Participate in the hospice program's quality assessment and performance improvement program
Requirements
Minimum 3 year of Hospice experience
Active RN license
Current BLS/BCLS Certification
Hospice case management experience preferred
Ability to provide care throughout the assigned service territory
