The nurse will travel throughout the assigned San Francisco-area territory to patients' homes/settings, admit patients to hospice, assess their condition and family needs, create and manage their care plans, coordinate with the hospice team, and conduct follow-up visits.

Job Description:

• Position: Hospice RN / RN Case Manager
• Location: San Francisco, CA
• Territory: San Francisco, Brisbane, Daly City, Colma, South San Francisco, Pacifica, and neighboring cities as needed
• Experience: Minimum 1 year of Hospice RN experience
• Specialty: Hospice Nursing

Key Responsibilities: 

• Admit patients to hospice services and complete comprehensive assessments.
• Assess the patient's physical, psychosocial, environmental, safety, and developmental needs.
• Develop an individualized plan of care based on patient and family needs.
• Document patient observations, assessments, and interventions accurately.
• Coordinate care with the multidisciplinary hospice team.
• Conduct follow-up visits as needed.
• Participate in the hospice program's quality assessment and performance improvement (QAPI) activities.
• Provide patient- and family-centered care throughout the hospice journey.

Seeking a Hospice RN Case Manager with 1+ year of hospice experience to provide compassionate field-based care throughout San Francisco and surrounding communities.
