🩺 Your Responsibilities
As an Extended Care RN, you will be responsible for providing comprehensive nursing care while serving as a strong advocate for residents.
Assessment & Care Planning
- Complete comprehensive admission histories and physical assessments.
- Perform ongoing assessments to identify changes in resident condition.
- Develop, implement, and evaluate individualized plans of care.
- Review and incorporate laboratory, imaging, and diagnostic information into care planning.
- Monitor residents for changes in nutritional, functional, and clinical status.
- Identify and appropriately report potential signs of neglect or abuse.
Clinical Care
- Provide safe, compassionate, and evidence-based nursing care.
- Administer medications and IV therapies within scope of practice and applicable policies.
- Recognize and respond appropriately to urgent and emergent changes in resident condition.
- Demonstrate strong clinical judgment and technical nursing competency.
- Accurately document nursing interventions, resident responses, and outcomes.
- Continuously evaluate resident progress and adjust care plans according to changing needs.
Resident & Family Advocacy
- Serve as a dedicated resident advocate.
- Partner with residents and families to support informed healthcare decisions.
- Identify resident and family education needs and provide appropriate education.
- Incorporate resident preferences, goals, and needs into daily care.
- Help residents achieve or maintain their highest practicable level of function.
- Support continuity of care throughout the resident's stay.
Teamwork & Leadership
- Collaborate with physicians and interdisciplinary healthcare professionals.
- Coordinate medical and nursing plans of care.
- Delegate responsibilities appropriately based on resident needs and staff competency.
- Supervise, coach, counsel, and evaluate assigned CNA staff.
- Promote teamwork, communication, safety, and quality improvement.
- Serve as a professional role model for other members of the healthcare team.
Discharge & Regulatory Responsibilities
- Identify and document discharge and transition-of-care needs.
- Coordinate transfer and discharge activities.
- Provide and document instructions related to medications, activity, wound care, and follow-up appointments.
- Maintain knowledge of CMS regulations, Medicare skilled-care requirements, and MDS criteria.
- Ensure documentation accurately reflects resident condition and supports the need for continued care.
- Participate in quality improvement initiatives and maintain professional standards.
🎓 Required Qualifications
- ASN / Associate Degree in Nursing from an accredited nursing program.
- Current RN license in the state of practice.
- BLS certification from the American Heart Association (AHA).
- Ability to read, write, and communicate effectively for clinical documentation.
- Strong clinical assessment, organization, communication, and prioritization skills.
- Ability to work effectively both independently and within an interdisciplinary team.

💪 Physical Requirements

The position may require frequent lifting/carrying of objects up to approximately 50 lbs. and the ability to safely handle heavier objects as required.
