Job description
Wound Care Nurse
We are seeking a compassionate Wound Care Nurse to oversee our resident skin and wound care program. Working under the Director of Nursing, you will deliver specialized, empathetic care to our frail elderly residents, driving high standards for healing and prevention across our facility.
Key Responsibilities
- Clinical Care: Conduct head-to-toe skin assessments for new admissions within 24 hours. Manage treatments for Stage 2–4 pressure injuries, surgical sites, ostomies, and Wound VACs.
- Assessments & Care Plans: Complete Braden Scales and risk assessments on admission, quarterly, and as needed. Initiate and update individual skin care plans and EHR logs.
- Interdisciplinary Collaboration: Lead weekly wound rounds with external consultants, participate in Medicare/PPS clinical meetings, and supply MDS Section M data.
- Program Oversight: Monitor daily EHR dashboards for compliance, assist with QAPI studies, manage specialty mattress inventory, and educate staff, residents, and families on prevention.
Qualifications
- Active, unencumbered Michigan nursing license (RN preferred) and current CPR certification.
- Graduate of an accredited nursing program.
- Prior wound care experience required; Wound Care Certification (WCC/CWCN) preferred.
- Strong proficiency in electronic medical record clinical documentation.
- Clean background and drug screen; strictly zero history of abuse, neglect, or mistreatment in a healthcare setting.
