We are looking for a passionate RN Inpatient Care Coordinator to join our dynamic team! - **Key Responsibilities:** - Manage patient progression of care and ensure evidence-based protocols are followed. - Monitor hospitalization necessity and quality of care for patients. - Collaborate with the healthcare team to develop personalized treatment plans. - Educate patients and families on healthcare benefits. - Perform utilization review functions and collaborate with regulatory agencies. - Facilitate educational programs and advise on regulations affecting patient care. - **Essential Qualifications:** - Requires an active nursing license in Alaska (must be held at the time of submission). - Bachelor’s Degree in Nursing. - 8 years of clinical care or nursing experience, with 3 years in chart review or risk management. - Case Management certification preferred (e.g., NCQA, CCMC). - **Skills and Abilities:** - Strong knowledge of medical terminology, anatomy, physiology, and disease concepts. - Proficient in conducting and reviewing medical records. - Excellent written and oral communication skills. - Ability to assess patient situations and take effective action. - **Work Environment:** - Collaborate with a team that prioritizes patient care and effective treatment plans. - Engage in a culture of continuous improvement and clinical excellence. - **Additional Features:** - Fingerprinting and background checks required prior to start date to ensure a smooth onboarding process.
