Requirements:
- Minimum 2 years of acute care Case Management experience
- Recent Care Coordination experience within the last 3 months (Required)
- Travel Case Manager experience required (Hard Stop)
- Active California RN License (pending accepted if active by start)
- Epic EMR experience required (Hard Stop)
- Experience with:
- Care Coordination & Discharge Planning
- DME, SNF, and ambulance arrangements
- HMOs, IPAs, family meetings, and treatment planning
- Community resources, mental health, homelessness, foster/adoption services
