**Join Our Team as a CO - RN Case Manager!** **Job Description:** - Full-Time Position - Weekend Coverage: 5 hours for one Saturday each month; take 5 hours off on one weekday **Position Overview:** - Be a key player in enhancing the CWHS Case Management Plan through development, implementation, and evaluation. - Engage in a collaborative process that assesses, plans, implements, coordinates, monitors, and evaluates the health and human services needs of clients/patients. - Serve as an advocate, communicator, collaborator, resource manager, and educator within an interdisciplinary team. **Key Responsibilities:** - Maintain and enhance the case management system by interacting with patients, physicians, and healthcare providers to achieve clinical, operational, and financial goals for patient populations. - Document Case Management plans clearly and concisely. - Assist physicians, patients, families, staff, and payers in delivering effective, appropriate, and cost-efficient patient care. - Attend and actively participate in departmental and interdepartmental meetings related to Case Management/Social Services functions. - Collaborate with payers and review organizations to ensure optimal utilization of patient care resources. - Document all contacts and information related to the patients' case management process accurately. - Educate physicians and staff on appropriate level of care and utilization issues. - Stay updated on Case Management/Social Services, discharge planning, and nursing issues through ongoing education. - Propose and implement new methods and techniques to improve processes. - Engage in professional activities for continuous development and to advance the profession. - Possess knowledge of the continuum of care, discharge planning, and utilization processes. **Qualifications:** - Bachelor’s Degree in Nursing (Master’s preferred) - Minimum of three (3) years clinical experience in a hospital setting - Minimum of five (5) years case management experience in a hospital setting - Certification in case management preferred (or willingness to obtain within two years) - Current unrestricted Registered Nurse license in the State of Colorado - Current BLS certification **Why Catalytic Solutions (CatSol):** At CatSol, we connect dedicated therapists with rewarding assignments across the country.
