**Exciting Opportunity for a RN Case Manager!** **Job Description:** - Are you ready to make a meaningful impact in patient care? - Join our dynamic team as a RN Case Manager and play a vital role in coordinating management of care. - Ensure optimum utilization of resources and service delivery while maintaining compliance with external review agencies. - Provide ongoing support and expertise through: - Comprehensive assessments - Care planning - Plan implementation - Overall evaluation of individual patient needs - Enhance the quality of patient management and satisfaction, promoting continuity of care and cost-effectiveness. - Integrate functions of case management, utilization review and management, and discharge planning. - Serve as a patient advocate through resource utilization, discharge planning, and addressing holistic patient needs. **Requirements:** - Must possess BLS certification (from the American Heart Association). - Minimum of 2 years recent RN Case Management experience, particularly in LTAC or STACH settings. - Experience with MCG and discharge planning across acute care settings. - Strong collaboration skills within an Interdisciplinary Team (IDT). - Active CA RN license is required. **Shift Details:** - Monday to Friday, 8:30 AM - 5:00 PM. **Why Catalytic Solutions (CatSol):** At CatSol, we connect dedicated therapists with rewarding assignments across the country.
