Monday-Friday, 8am-4:30pm

Every third weekend rotation

Holiday rotation

Additional tools: Wellsky, Careport/PASSR/Teams, Laptop, Phone (ascom)

Must Have Skills/Experience:  

Discharge planning and care coordination, case management minimum 5 years

Preferred Skillset/Experience: 

More than 5+ years nursing practice in hospital
