• Schedule: Monday through Friday, 8:00 AM to 5:00 PM
• Weekly Estimated Pay: $1,880 to $1,980
• Patient Volume: Approximately 5–8 patients per day
• Productivity Expectation: 30 points per week
• Travel Territory: Up to 75 miles from the assigned care center, including surrounding local communities
• Weekend/On-Call: Weekend rotation and on-call coverage may vary by assignment
• Documentation System: HomeCare HomeBase; OASIS proficiency required for RNs

Job Requirements

• Current Registered Nurse (RN) license in hand
• Minimum 1 year of adult home health experience or 1 year of hospice experience, depending on specialty
• Recent experience with HomeCare HomeBase within the last 6 months preferred
• OASIS proficiency required for RNs
• Current CPR/BLS certification
• Reliable transportation, valid driver’s license, and auto insurance
• Willingness to travel daily to patient homes across the assigned service area
• Comfortable working independently and managing a full home health caseload
• Ability to complete documentation within 24 hours of each visit
• Open to on-call rotations and weekend coverage as needed

Responsibilities

• Provide skilled nursing care in the home setting based on physician orders and care plans
• Complete start-of-care, wound care, follow-up visits, and other home health visits as assigned
• Assess patient needs, implement nursing interventions, and update care plans accordingly
• Educate patients and families on treatment plans, safety, and self-care techniques
• Coordinate care with the interdisciplinary team to support positive patient outcomes
• Supervise and evaluate home health aides and LPN/LVNs when applicable
• Travel between patient homes within the assigned territory and attend required meetings
• Document all care thoroughly and accurately in the EMR within required timelines
• Maintain compliance with productivity standards and assignment expectations
• Deliver compassionate, high-quality care that helps patients remain healthy at home
