Southwest Philadelphia | Full-Time | $47–$59/hr

Home Health RN Responsibilities

• Perform comprehensive patient assessments and ongoing skilled nursing visits.
• Develop, implement, and update individualized plans of care.
• Administer medications, wound care, IV therapy, and other skilled nursing treatments as ordered.
• Monitor patient progress, identify changes in condition, and communicate updates with physicians and the interdisciplinary care team.
• Educate patients and caregivers on disease management, medications, treatments, and safety in the home.
• Complete accurate, timely clinical documentation in accordance with agency, state, and federal guidelines.
• Coordinate care and facilitate appropriate referrals to ensure optimal patient outcomes.

 
Requirements

• Active Pennsylvania RN License
• Minimum of 2 years of Home Health or Acute Care RN experience
• Reliable transportation
• BSN preferred
