Job Requirements

• Active Registered Nurse license in good standing
• At least 1 year of bedside clinical experience required; LTAC or acute care nursing experience preferred
• BLS certification required prior to first shift
• ACLS certification required prior to first shift
• Strong assessment, documentation, and care coordination skills
• Ability to work independently while contributing to a collaborative healthcare team
• Comfortable caring for medically complex patients in a fast-paced LTAC environment

Responsibilities

• Perform comprehensive nursing assessments within the required timeframe and develop individualized plans of care
• Administer direct patient care using the nursing process: assess, plan, implement, and evaluate
• Document assessments, interventions, and patient responses accurately and promptly in the medical record
• Coordinate care with physicians, charge nurses, and interdisciplinary team members to support continuity of care
• Support discharge planning and patient and family education from admission through discharge
• Supervise and direct ancillary nursing personnel as appropriate
• Promote a safe environment for patients, visitors, and staff by following established policies and procedures
• Contribute to a positive, efficient unit culture through teamwork, communication, and professionalism
• Participate in ongoing professional development and continuous improvement initiatives
