Schedule
- Shift: Monday–Friday, 8:00 AM–5:00 PM
- Hours: 40 guaranteed hours/week
- Weekend/On-call: Occasional weekend backup and on-call
- Contract: 12 weeks
- Setting: Hospice care in patients’ homes, nursing homes, group homes, or assisted living facilities
Requirements
- Registered Nurse (RN)
- Strong clinical assessment and critical-thinking skills
- Hospice nursing experience/knowledge preferred based on the role description
- Ability to provide symptom management
- Ability to educate patients and caregivers
- Ability to work independently with an assigned patient caseload
- Ability to participate in and lead an interdisciplinary care team
Responsibilities
- Provide compassionate hospice care to patients and families
- Manage an assigned caseload of hospice patients
- Perform comprehensive patient assessments
- Provide symptom management and monitor changes in condition
- Develop and support individualized goals of care
- Educate patients and caregivers on disease and care needs
- Build caregiver confidence in providing care at home
- Coordinate with the interdisciplinary team
- Provide care across various settings: private homes, nursing homes, group homes, and assisted living facilities
- Provide occasional weekend/on-call coverage when needed
