Job Summary:
Home Health Hospice Nurse - Clarksburg, West Virginia. This position involves providing hospice care to terminally ill patients within their homes or nursing facilities. The role requires delivering compassionate care, performing daily visits, and adhering to hospice standards in a home health setting.
Job Details:
- Location: Clarksburg, West Virginia (serving Harrison, Barbour, Upshur, Lewis, Taylor, Doddridge, Wetzel counties)
- Schedule: Days, 8-hour shifts, Monday through Friday
- Minimum Hours Per Week: 40 hours
- Start Date: 09/07/2026
- End Date: 12/07/2026
- Required Qualifications: BLS (American Heart Association); West Virginia nursing license; minimum of 2 years of hospice or home health nursing experience; active EPIC electronic medical record proficiency
- Preferred Qualifications: Experience with terminally ill patients and hospice care
- Float: Float rotation may be required
- Additional Information: Must have at least 1 year of hospice experience, providing about 5-6 patient visits per day in an outpatient and inpatient hospice setting. No weekend or holiday requirements. Uniform color is Royal Blue.
- Responsibilities include caring for patients with end-stage diseases such as cancer, heart disease, lung disease, and dementia in their homes or nursing facilities, following facility protocols and electronic health records.
