Pride Health is actively hiring a Registered Nurse (RN)  to join our client's growing Home Health team.

Location: Harahan, LA, 70123; Coverage Areas: Greater New Orleans Area, LA

Schedule: Monday–Friday, 8:00 AM–4:30 PM
On-Call/Weekend: None
Productivity: 30 points/week
 

Compensation:

• Travel Pay Package: Up to $2,550/week gross, including weekly non-taxable stipend*
• Mileage Reimbursement: $0.48/mile

Job Summary:

The Registered Nurse (RN) – Home Health providers and directs skilled nursing care to patients in their homes as prescribed by the physician and in compliance with applicable federal and state regulations, payer requirements, and agency policies.

The RN works closely with patients, families, physicians, and interdisciplinary care teams to ensure high-quality, coordinated, and patient-centered care.

Key Responsibilities:

• Provide skilled nursing care to patients in their homes according to physician orders and the established plan of care.
• Coordinate the total plan of care with physicians, therapists, agency staff, patients, families, and community resources.
• Perform initial, comprehensive, and follow-up nursing assessments at designated time points.
• Determine the primary focus of care and develop an individualized plan of care within state-specific guidelines.
• Monitor patient condition and identify changes requiring intervention or physician notification.
• Ensure accurate, complete, and timely documentation within the Point of Care/EMR system.
• Collaborate with the interdisciplinary team to promote continuity and quality of patient care.
• Educate patients and caregivers regarding medications, disease management, treatments, safety, and home care procedures.
• Ensure patients meet home health eligibility and medical necessity requirements as defined by applicable payer guidelines.
• Follow all applicable federal, state, local, and agency regulations, policies, and procedures.
• Maintain patient confidentiality and uphold professional standards of nursing practice.

Requirements

• Current Registered Nurse (RN) license in the state of practice.
• Current CPR certification.
• Valid driver's license.
• Current vehicle insurance.
• Access to a dependable vehicle or reliable transportation.
• Ability to provide home health nursing services throughout the Greater New Orleans Area.
• Strong assessment, clinical documentation, communication, and care coordination skills.
