Pride Health is actively hiring a Registered Nurse (RN) to join our client's growing Home Health team.
Travel Pay Package: Up to $2,500/week gross, including weekly non-taxable stipend*
Location: Denver, CO 80222
Coverage Area: Denver Metro Area
Mileage: ($0.48/Mile) Reimbursed between patient visits, from the first patient to the last patient
Schedule: Monday–Friday: 8:00 AM–4:30 PM
On-Call: Rotating schedule, including weekends
Productivity: 30 points per week
Key Responsibilities:
- Provide skilled nursing care to patients in their homes, including wound care, catheter management, medication management, and lab services.
- Perform Start of Care (SOC) visits and OASIS assessments, ensuring accurate and timely documentation.
- Serve as an RN case manager, coordinating patient care and collaborating with physicians and the interdisciplinary care team.
- Provide patient/family education on medications, disease management, treatment plans, and home safety.
Required Qualifications:
- Active Registered Nurse (RN) license.
- Previous Home Health RN experience.
- Hands-on experience with OASIS and Start of Care (SOC) assessments.
- Strong clinical, communication, organizational, and documentation skills.
Preferred Skills:
- Experience with wound care, catheter management, medication management, and home health care management.
- Ability to work independently and manage a caseload effectively.
- Strong patient education and care coordination skills.
