Job Details
- Schedule: Days, 8 hours per day, minimum of 40 hours per week, starting on 09/08/2026
- Required Qualifications: Minimum of 1 year of relevant experience, including Adult Home Health, Admission Assessment, Discharge/Discharge due to safety or Non-compliance, Recertification, Resumption of care, Routine visits, Start of Care, Wound care, Medication Assessment/Management, Pain assessment & management, Utilization of pain scales (verbal, nonverbal), Starting and maintaining IVs, Phlebotomy skills, Infection Control Precautions, Medication Administration (Intramuscular, Intravenous infusions, Subcutaneous), OASIS experience required for staff completing Start of Care, proficiency in HCHB (Homecare Homebase), license in-hand, active drivers license, and personal transportation.

- Additional Information: No call or weekend duties, no overtime, and no on-call rotation unless discussed during interview. Timely communication and documentation are mandatory. Former permanent employees must be separated for at least six months before being eligible to reapply as travelers.
