Shift:Day 5x8-Hour (08:00 - 17:00)
Home Health Position – Job Details
- Work Setting: Home visits with periodic office meetings
- Transportation: Reliable transportation required
- Driver’s License: Current/valid driver’s license required
- Insurance: Agency-required liability insurance required
Patient Volume
- Average Patients/Day: 5–8 patients
- Types of Cases: Start of Care (SOC), wound care, follow-up visits, and other home health visits
Productivity / Points
- Points: 30 points per week
- Each visit/activity has a different point value, including:
- Patient visits
- Documentation
- Travel
- Other assigned activities
- Contractors are typically assigned 5–10 patients per day, averaging approximately 6–8 points per day
Experience Requirements
- Minimum 1 full year of recent Adult Home Health experience, OR
- 1 full year of Hospice experience, depending on the specialty
- First-time travelers: ❌ Not accepted
- Must have recent specialty experience
Schedule
- Weekend Requirement: Varies by location and profession; discuss during the interview
- On-Call: Varies by location and profession; discuss during the interview
Certifications
- CPR/BLS: Required
Territory / Travel
- Must be willing to travel up to 75 miles from the home clinic
- Mileage reimbursement: IRS standard mileage rate
- Mileage is reimbursed from patient home to patient home
- Travel from home to the first patient and last patient back home is not billable/reimbursable
