As a Case Manager RN, you will coordinate patient care, manage discharges, and collaborate with multidisciplinary teams. You will work within a Magnet facility that prioritizes pediatric care, ensuring that patients receive the highest quality of support. This role requires flexibility to meet department needs, including weekend coverage. Your expertise in Epic and acute care will be essential as you navigate the complexities of patient management.
