Schedule Notes:
Schedule: 8:00 AM – 4:00 PM Hospital experience required, including experience with discharge planning and placement in rehabilitation facilities Minimum 2 years of experience with acute inpatient hospital discharge planning Rotating weekends: Every 4th or 5th weekend Holidays: One summer and one winter holiday Experience with subacute patients required
Duties:
Coordinates patients through the acute care hospitalization Manages all aspects of discharge planning/transition process Performs utilization management and quality screenings In- Patient Case Management is required
