RN Case Manager – Simplified

Position: RN Case Manager
Facility: Acute Care Hospital – Magnet, Level II Trauma
Location: Alaska
Start: ASAP
Contract: 13 weeks
Shift: Days, Monday–Friday, 8 hours/day
Weekend: Every other weekend / 4 weekend days per 4 weeks
Call: No

Requirements

• AK RN License – required; must be in hand for Auto Offer
• Acute Care Hospital experience – REQUIRED
• Epic experience – REQUIRED
• BLS – preferred, not required
• ACM certification – preferred
• Minimum 1–2 years RN/Case Management experience
• Must have case management/discharge planning experience
• Behavioral health-only experience does not qualify
• Trauma Level II experience preferred
• Travel experience not required

Main Responsibilities

• Case management and discharge planning
• Care coordination
• Assess patient needs and develop care plans
• Utilization review and medical necessity
• Prior authorizations
• Continued-stay reviews
• Work with physicians, social workers, therapy, and other departments
• Use Epic for documentation
• Apply InterQual/Milliman guidelines and CMS requirements
