RN Case Manager – Simplified
- Position: RN Case Manager
- Shift: Monday–Friday, 8:00 AM–5:00 PM
- Hours: 40 hours/week
- Contract: 13 weeks
Requirements
- RN Case Management experience: Minimum 2 years recent experience
- Must have experience in:
- LTAC or STACH
- MCG
- Discharge planning in acute-care settings
- Strong collaboration with the Interdisciplinary Team (IDT)
- Texas RN license or Compact/Multi-State RN license
- BLS through American Heart Association (AHA) — must be attached to submission
Main Responsibilities
- Assess patients and coordinate their care.
- Create and implement care plans.
- Coordinate discharge planning.
- Ensure proper use of healthcare resources.
- Work with doctors, nurses, social workers, and other healthcare professionals.
- Advocate for patients and consider their individual needs and preferences.
- Help improve patient satisfaction, continuity of care, and cost effectiveness.
- Perform utilization review and case management.
