RN – Care Manager (Home Health)
13-Week Contract
Shift: Days | 8:00 AM – 4:30 PM
Mileage: Paid in addition to the weekly pay
Requirements:
- Active CA RN License
- EPIC experience REQUIRED
- Home Health or acute care experience preferred
- BLS required
- Valid Driver’s License
- Must be 21+ years old
- Reliable vehicle & current auto insurance required
Position Highlights:
- Manage a patient caseload in a home health setting
- Conduct initial and ongoing patient assessments
- Develop and update individualized plans of care
- Provide skilled nursing care and patient/family education
- Coordinate care with other disciplines
