Job Summary:
Hospice Registered Nurse (RN) Night Shift located in a healthcare setting providing end-of-life care. The role involves conducting patient assessments, developing and implementing care plans, ensuring compliance with hospice regulations and insurance reimbursement guidelines, and collaborating with interdisciplinary teams. The position requires a dedicated RN with experience in hospice care, capable of maintaining a safe environment and providing patient and family education.
Job Details:
- Schedule: 12-hour night shifts from 4:00 PM to 8:00 AM, three shifts per week, with a minimum of 36 hours per week. On-call coverage required for Morris and Sussex Counties. Weekend work every 3 weeks with designated days off, and participation in one summer/winter holiday.
- Required Qualifications: Must have two (2) years of experience as RN in home hospice environment, experience using an electronic medical record (EMR), excellent critical thinking, and communication skills.
- Preferred Qualifications: Three (3) or more years of RN experience, specifically within a hospice agency or organization.
- Additional Information: Responsible for patient assessments, care planning, medication reconciliation, infection control, and participation in performance improvement activities. The role involves educating patients and families using age-appropriate methods, supervising home health aides, and attending bi-weekly interdisciplinary team meetings. Provides mentorship to new employees, cross-trains in new skills, and assists in agency growth efforts by orienting new staff.
