Job Summary:
Home Healthcare Professional - Local area. This position involves delivering comprehensive in-home nursing care to adult and geriatric patients across multiple counties. The role requires assessment, coordination, and implementation of personalized care plans while fostering collaborative relationships with patients, families, and healthcare providers. The position offers an opportunity to provide holistic, patient-centered care in a community setting, with flexibility to float to various counties as needed.
Job Details:
- Schedule: 8-hour shifts, minimum of 40 hours per week, starting 09/08/2026 and ending 12/08/2026
- Required Qualifications: RN licensure in California, BLS certification, minimum of 2 years of home healthcare experience, proficiency with Epic charting system, valid CA State License, uploaded Nursys verification within the last 2 weeks, and applicable certifications with valid expiration dates
- Preferred Qualifications: Previous experience in adult and geriatric home health care, experience with multidisciplinary collaboration, and familiarity with patient assessment and care planning
- Float: Float: Float rotation may be required to areas that need help
- Additional Information: The role involves providing direct patient care, patient/family education, and transitions of care. Candidates should have skills in blood and blood products administration, IV management, wound care, medication management, and use of clinical assessment tools.
