Job Opportunity – Registered Nurse, Case Management – Santa Rosa, CA 95405
Start Date: September 8, 2026
Location: Santa Rosa, California
Schedule: Day shift, 5x8-hour shifts (08:00 - 16:30), 2 weekends every 6 weeks. No call required. 40-hour orientation provided. Caseload of approximately 20 patients.

Requirements:
- Active California RN license or Compact license (must be in hand at submission)
- Minimum 2 years of case management experience required
- Current work history within the past 2 years (hard requirement)
- BLS certification required
- Epic charting system experience required
- Previous travel experience required; first-time travelers accepted with clinical approval
- Community hospital, LTAC, and Trauma Level I/II experience preferred
- Ability to independently arrange DME, SNF placement, and ambulance transport
- Experience with discharge planning, care coordination, and utilization of InterQual criteria
- Competency in organizing and participating in family meetings and treatment plan development
- Knowledge of HMOs, IPAs, and insurance authorization processes
- Two professional references covering one year within the last three years
