Job Opportunity – Registered Nurse, Case Manager – Burbank, CA 91505

Start Date: August 31, 2026

Location: Burbank, California

Schedule: Day shift, 5x8-hour shifts (08:00 - 16:30). Typically one full weekend per 4 weeks, subject to department needs. Case load up to 25 patients. 40 hours of orientation provided.

Highlights:

• Acute care facility setting with focus on care coordination and discharge planning
• Work with adult and geriatric patient populations

• 91-day assignment with potential for extension

Requirements:

• Active California RN license (pending licenses accepted but must be active by start date)
• Minimum 2 years of acute medical Care Manager experience performing care coordination and discharge planning
• Epic charting system experience required
• Experience with HMOs, IPAs, and managed care organizations
• Proficiency in admission criteria, medical necessity determination, discharge planning, DRG, CPT coding, and HIPAA guidelines
• Strong skills in needs assessment, plan of care development, and disease management
• Two professional references covering 1 year within the last 3 years
• Travel nursing experience preferred
