Position Overview

Seeking an experienced Case Manager (RN or Social Worker) to coordinate patient care, facilitate discharge planning, and collaborate with interdisciplinary teams to ensure safe, efficient transitions of care. The ideal candidate will have recent acute hospital case management experience with a strong background in rehabilitation and care coordination.

Requirements

• Registered Nurse (RN) or Licensed Social Worker (LCSW, LCSW-C, LMSW) accepted
• Active Maryland RN or MD Social Work License (LMSW/LCSW/LCSW-C)
• BLS Certification
• Minimum 2 years of Case Management experience
• Acute Hospital experience required
• Experience in acute rehabilitation, discharge planning, interdisciplinary collaboration, and care coordination
• Current hospital Case Management experience on a Medical-Surgical unit preferred

Schedule

• Monday–Friday | 8:00 AM – 4:30 PM (5×8s)
• ASAP Start Required

Key Responsibilities

• Coordinate patient care and discharge planning from admission through discharge.
• Collaborate with physicians, nurses, therapists, and interdisciplinary teams.
• Assess patient needs and develop individualized transition-of-care plans.
• Arrange post-acute services, rehabilitation, and community resources.
• Ensure compliance with hospital policies and payer requirements while promoting quality patient outcomes.

Seeking an experienced RN or Social Worker Case Manager with acute hospital experience in discharge planning, rehabilitation, and care coordination for a Monday–Friday assignment in Maryland.
