In this role, you will provide personalized, one-on-one behavioral health nursing care to patients in their homes. As a Behavioral Health Registered Nurse, you will manage an assigned caseload and coordinate individualized care for patients with behavioral health and coexisting medical needs.
Your responsibilities will include:
Conducting comprehensive assessments to identify each patient’s physical, behavioral, psychosocial, environmental, and safety needs
Developing, implementing, coordinating, and evaluating individualized plans of care
Completing initial evaluations and reassessing each patient’s nursing needs during subsequent visits
Providing skilled behavioral health nursing visits based on the patient’s plan of care
Monitoring changes in the patient’s mental, emotional, and physical condition
Educating patients and caregivers about diagnoses, medications, symptom management, coping strategies, and available community resources
Supporting medication adherence and identifying potential medication concerns or side effects
Recognizing changes in condition, safety risks, or potential crises and communicating concerns promptly to the appropriate clinical team members
Collaborating with physicians, therapists, caregivers, facilities, and other members of the interdisciplinary care team
Revising the plan of care when patient needs or circumstances change
Providing patients and families with the education, resources, and support needed to promote stability and independence at home
Supervising and documenting care provided by behavioral health aides and LPNs, when applicable
Maintaining timely, accurate, and complete clinical documentation in accordance with agency policies and regulatory requirements
Graduate of an accredited Certificate, Diploma, Associate, or Bachelor’s Degree nursing program
Current, unrestricted Registered Nurse license in the state of Massachusetts
At least one year of nursing experience in a clinical care setting
Behavioral health, psychiatric nursing, home health, or hospice experience preferred
Associate or Bachelor’s Degree in Nursing preferred
Homecare HomeBase experience preferred
Strong assessment, communication, organization, and care-coordination skills
Ability to work independently while collaborating effectively with an interdisciplinary team
Willingness and ability to travel approximately 50% within the Worcester-area coverage territory
Dependable vehicle, valid driver’s license, and current automobile insurance in accordance with state requirements
Sit, stand, bend, lift and move intermittently and be able to lift 50–100 lbs.
