Registered Nurse – Behavioral Health Home Visits

$5,000 Sign-On Bonus | Worcester Area

Elara Caring is growing, and we are seeking a compassionate and experienced Behavioral Health Registered Nurse to join our supportive Worcester-area team.

In this role, you will provide personalized, one-on-one behavioral health nursing care to patients in their homes. What You’ll Do

As a Behavioral Health Registered Nurse, you will manage an assigned caseload and coordinate individualized care for patients with behavioral health and coexisting medical needs.

Your responsibilities will include:

  • Conducting comprehensive assessments to identify each patient’s physical, behavioral, psychosocial, environmental, and safety needs

  • Developing, implementing, coordinating, and evaluating individualized plans of care

  • Completing initial evaluations and reassessing each patient’s nursing needs during subsequent visits

  • Providing skilled behavioral health nursing visits based on the patient’s plan of care

  • Monitoring changes in the patient’s mental, emotional, and physical condition

  • Educating patients and caregivers about diagnoses, medications, symptom management, coping strategies, and available community resources

  • Supporting medication adherence and identifying potential medication concerns or side effects

  • Recognizing changes in condition, safety risks, or potential crises and communicating concerns promptly to the appropriate clinical team members

  • Collaborating with physicians, therapists, caregivers, facilities, and other members of the interdisciplinary care team

  • Revising the plan of care when patient needs or circumstances change

  • Providing patients and families with the education, resources, and support needed to promote stability and independence at home

  • Supervising and documenting care provided by behavioral health aides and LPNs, when applicable

  • Maintaining timely, accurate, and complete clinical documentation in accordance with agency policies and regulatory requirements

What You’ll Need
  • Graduate of an accredited Certificate, Diploma, Associate, or Bachelor’s Degree nursing program

  • Current, unrestricted Registered Nurse license in the state of Massachusetts

  • At least one year of nursing experience in a clinical care setting

  • Behavioral health, psychiatric nursing, home health, or hospice experience preferred

  • Associate or Bachelor’s Degree in Nursing preferred

  • Homecare HomeBase experience preferred

  • Strong assessment, communication, organization, and care-coordination skills

  • Ability to work independently while collaborating effectively with an interdisciplinary team

  • Willingness and ability to travel approximately 50% within the Worcester-area coverage territory

  • Dependable vehicle, valid driver’s license, and current automobile insurance in accordance with state requirements

  • Sit, stand, bend, lift and move intermittently and be able to lift 50–100 lbs.
