Position Overview
Seeking a compassionate and experienced Hospice Registered Nurse (RN) Case Manager to provide high-quality end-of-life care to patients in their homes and long-term care facilities. The RN Case Manager will coordinate and manage all aspects of patient care, serving as the primary clinical resource for assigned hospice patients and their families. This role requires strong clinical judgment, excellent communication skills, and the ability to work independently while delivering patient-centered hospice care.
Schedule & Work Details
- Schedule: Full-time, 40 hours per week
- Available Hours: Monday through Friday, 8:00 AM - 6:00 PM
- Coverage Area: 60-mile radius from the office location
- On-Call Requirement: Backup on-call one evening/night during the week
- Weekend Requirement: Backup on-call weekend rotation every 4-6 weeks
- Patient Care Setting: Patient homes and healthcare facilities
- Mileage Reimbursement: Reimbursed between patient visits; mileage from home to first visit and last visit to home is not reimbursed
- Dress Code: No designated scrub color
- Pay Rate: $51.00 - $53.00 per hour
Key Responsibilities
- Serve as the primary RN Case Manager for an assigned hospice patient caseload.
- Conduct comprehensive nursing assessments and ongoing evaluations of patient condition, symptom burden, and care needs.
- Develop, implement, and update individualized hospice plans of care in collaboration with physicians and the interdisciplinary team.
- Provide skilled nursing care focused on symptom management, pain control, comfort measures, and quality of life.
- Coordinate care for patients residing in private homes, assisted living facilities, and skilled nursing facilities.
- Educate patients, families, and caregivers regarding disease progression, medication management, hospice philosophy, and end-of-life care.
- Communicate patient status changes and care needs to physicians, hospice leadership, and interdisciplinary team members.
- Maintain accurate and timely clinical documentation in accordance with agency policies and regulatory requirements.
- Participate in interdisciplinary team meetings, care conferences, and case reviews.
- Coordinate services with hospice aides, social workers, chaplains, therapists, and other healthcare professionals.
- Respond appropriately to changes in patient condition and urgent care needs.
- Assist with admissions, recertifications, routine visits, and discharge planning as needed.
- Participate in backup on-call responsibilities according to the established rotation schedule.
- Ensure compliance with Medicare hospice regulations, agency policies, and professional nursing standards.
Required Qualifications
- Active and unrestricted Registered Nurse (RN) license.
- Strong clinical assessment and care coordination skills.
- Valid driver's license, reliable transportation, and proof of insurance.
- Ability to travel throughout a 60-mile service radius.
- Excellent communication, organizational, and documentation skills.
- Ability to work independently and manage a field-based caseload.
Preferred Qualifications
- Previous hospice nursing experience.
- Prior RN Case Manager experience.
- Experience providing care in both home and facility-based hospice settings.
- Strong end-of-life symptom management and pain management skills.
- Experience managing an independent hospice caseload.
- Travel nursing or extensive field-based healthcare experience.
- Compassionate, adaptable, and patient-focused approach to care.
