Position Overview

Seeking an experienced and compassionate Hospice Registered Nurse Case Manager (RNCM) to provide comprehensive end-of-life care to patients throughout the Columbia, South Carolina service area. The RNCM is responsible for managing assigned hospice patients, coordinating care across the interdisciplinary team, addressing patient medical needs, and ensuring accurate bedside documentation. This role requires strong communication skills, flexibility, and a commitment to delivering exceptional hospice care to patients and their families.

Schedule & Work Details

  • Coverage Area: Up to 55 miles from the office, primarily Richland and Lexington Counties, SC
  • Schedule: Full-time
  • Shift: 8:00 AM - 4:30 PM
  • Work Days: Monday through Friday
  • On-Call/Weekend Requirement: Yes, as needed
  • Mileage Reimbursement: Yes
  • Patient Care Setting: Patient homes and community settings
  • Pay Rate: $51.00 - $53.00 per hour

Key Responsibilities

  • Serve as the primary nurse case manager for an assigned hospice patient caseload.
  • Visit assigned patients a minimum of two times per week, or more frequently based on patient needs and plan of care requirements.
  • Assess, monitor, and address the medical, physical, and psychosocial needs of hospice patients.
  • Provide skilled nursing care focused on comfort, symptom management, pain control, and quality of life.
  • Complete documentation at the bedside whenever possible, ensuring accuracy, timeliness, and regulatory compliance.
  • Develop, implement, and update individualized plans of care in collaboration with physicians, nurse practitioners, and the interdisciplinary team.
  • Communicate effectively with patients, families, caregivers, physicians, nurse practitioners, and hospice team members regarding changes in condition and care needs.
  • Attend and actively participate in agency meetings, patient care conferences, and interdisciplinary group (IDG) meetings.
  • Prepare and present patient updates and care plans during IDG meetings.
  • Coordinate hospice services with social workers, chaplains, hospice aides, volunteers, and other members of the care team.
  • Educate patients and families regarding disease progression, symptom management, medications, hospice services, and available resources.
  • Respond appropriately to changes in patient condition and assist with adjustments to the care plan as needed.
  • Maintain compliance with Medicare hospice regulations, agency policies, and professional nursing standards.
  • Demonstrate flexibility in scheduling and caseload management based on staffing levels and patient care needs.

Required Qualifications

  • Active Registered Nurse (RN) license in South Carolina in good standing.
  • Previous hospice nursing experience required.
  • Strong clinical assessment and symptom management skills.
  • Ability to manage an independent hospice caseload.
  • Valid driver's license, reliable transportation, and ability to travel throughout the service area.
  • Excellent communication, organizational, and documentation skills.
  • Ability to collaborate effectively with interdisciplinary healthcare teams.

Preferred Qualifications

  • Experience with Homecare Homebase (HCHB).
  • Previous hospice case management experience.
  • Strong bedside documentation skills.
  • Experience presenting patient cases during interdisciplinary group meetings.
  • Adaptable, trainable, and able to work effectively in a fast-paced environment.
  • Positive attitude, strong work ethic, and willingness to support changing patient and agency needs.
  • Exceptional relationship-building skills with patients, families, and healthcare professionals.
