RN Care Delivery Manager – Clinical Leadership

HumanEdge Healthcare Staffing is seeking an experienced Registered Nurse (RN) Manager for an exciting clinical leadership opportunity with a leading healthcare organization in the Bronx.
This is an excellent opportunity for an experienced RN Supervisor, Nurse Manager, Home Care Clinical Manager, or Care Management RN who wants to move into a highly visible leadership position while maintaining a Monday–Friday schedule.

What You'll Do
As the RN Care Delivery Manager, you will oversee clinical care delivery and support nursing excellence across an interdisciplinary healthcare team.
You will:

Oversee the development and execution of individualized participant care plans
-
Lead and participate in Interdisciplinary Team (IDT) meetings
-
Coordinate care across nursing, providers, social services, rehabilitation, home care, and other disciplines
-
Supervise, coach, mentor, and evaluate clinical staff
-
Help manage staffing, assignments, and clinical coverage
-
Review clinical documentation and participant charts for quality and compliance
-
Collaborate with clinicians to review UAS assessments and ensure assessment accuracy
-
Monitor participant outcomes and make adjustments to plans of care as needed
-
Oversee incident follow-up, investigations, and required documentation
-
Support transitions of care between hospitals, rehabilitation facilities, nursing homes, home care, and community settings
-
Participate in interviewing, onboarding, staff education, performance management, and professional development
-
Ensure compliance with organizational standards and applicable state and federal regulations
-
Serve as a clinical leadership resource to staff and the interdisciplinary team
The client's position also includes the possibility of serving as Director of Patient Services (DPS) for the LHCSA, as assigned.
What We're Looking For
Required
-
Active and unrestricted New York State RN license
-
Nursing degree from an accredited nursing program
-
Supervisory or nursing management experience
-
At least 2 years of healthcare administrative/management experience

Experience in one or more of the following:

Home Care / CHHA

LHCSA

Managed Long-Term Care

PACE
-
Nursing Home / Long-Term Care

Acute Care

Medical-Surgical or Critical Care

Diagnostic & Treatment Center

Strong clinical assessment and care coordination skills

Excellent leadership, communication, documentation, and organizational skills

Experience working with interdisciplinary healthcare teams
The client specifically requires supervisory experience and identifies CHHA, LHCSA, acute care, medical-surgical, critical care, nursing home, and diagnostic/treatment clinic experience as relevant backgrounds.
